Abstract Details9/26/2014 | 4:30 PM - 6:00 PM | Retrospective testing of newborn dried blood spots (DBS) in symptomatic children to confirm a presumed diagnosis of congenital CMV infection (cCMV) Background- Children with disabilities resulting from congenital CMV infection (cCMV) may not be tested for CMV at birth. Dried blood spot (DBS) testing may aid the retrospective diagnosis of these children. Aims- To evaluate DBS testing for detection of cCMV for infants that received urine or saliva-based testing within 3 weeks of birth, and and to the presence of clinical symptoms consistent with CMV infection for infants who did not receive CMV testing at birth Methods-Children referred for evaluation of possible congenital CMV infection were included in the study. Urine, saliva and plasma samples were sent for CMV testing by culture or DNA PCR testing, as available, at time of presentation. DBS samples obtained at 2 days and 2 weeks of age were retrieved, with written parental permission, from the Texas Department of Health State Laboratory. DNA was eluted by simple, low-cost thermal shock method and extracted DNA was analyzed by PCR targeting the viral immediate early region. Results-Of 36 infants tested to date, 9 were virologically confirmed to have c CMV infection and all 9 also were positive for CMV DNA on DBS testing, 4 infants were confirmed not to have cCMVand also tested negative for CMV by DBS testing. Of remaining 23 infants, whose congenital CMV status could not be confirmed had CMV DBS testing compatible with their clinical symptoms. Conclusions-Retrospective testing of DBS for CMV DNA will likely provide useful information to the clinician for infants that missed CMV testing at birth.
