Method of Payment
Full payment in U.S. currency is required with registration. The following forms of payment are accepted:

  • Credit Cards: Visa, MasterCard, American Express, Discover, Diners Club (Must be entered online or called in. For your security - do not fax, email, or mail your credit card number.)
  • Checks must be made payable to Utah State University. (Must be sent by mail with registration form.)

Confirmation & Receipt
Registrants will receive an immediate confirmation and receipt via email.

Cancellation Policy
Refunds will be made to those registrants who must cancel, less a 50% processing fee. Written cancellation requests must be post- marked on or before August 1, 2018. No refunds will be made after that date. Substitutions are welcome at no charge (if additional payment processing is required, a $25 processing fee will apply).

Accommodations for Persons with Disabilities
Reasonable accommodations are available for persons with disabilities. Notice of any special needs must be made by August 15, 2018. When you register, you will be asked to identify your needs, and you will be contacted for additional information.

Dietary Requirements
Medical and religious dietary requirements will be accommodated if requested by August 15, 2018. When you register, you will be asked to identify your specific dietary requirements.

Scholarships:

Family Scholarship Application

Background and Selection Criteria:

One of the purposes of the conference is to foster connections between families and professionals in order to raise awareness of CMV. The planning committee for the 2018 CMV Public Health and Policy Conference is pleased to announce that a limited amount of scholarships will be available for parents to attend the conference.

Scholarship amounts:
  • $250 for a family member that lives less than 200 miles from Burlington, VT
Scholarships will be prioritized based on the following criteria:
  • Those who have not previously attended a CMV Public Health and Policy Conference
  • Financial need
  • Given to one member of each family before awarding a second scholarship within the same family

Scholarship recipients will also be asked to provide information about their contributions and plans for future contributions to CMV public health and policy activities. Follow-up surveys will be sent to scholarship recipients, within three months post conference, to learn about how they have applied what they learned at the 2018 CMV Public Health and Policy Conference.

The deadline for scholarship applications is June 15, 2018.
